Created a FREE php form that
automatically sorts, post and redirects to different
locations based on the form results. You can do it without
any coding knowledge!
You decide where each of the three
responses are posted. Additionally, you decide where each
category will be redirected after submission. You can
create different "Thank You" (or "I'm Sorry" for
negative results) pages for your form / survey
results.
Examples of usage,
You can create a lead generation form
that will sort "HOT Prospects" and "Likely Prospects" from
"Unlikely". Send the "Hot Prospects" to a live operator to
to close the deal immediately. Send the "Likely" to a
database for auto responder follow up. Send the "Unlikely
Prospects" to another database and possibly sell
it.
You can create a customer service survey.
Customers that were happy with all aspects of your
customer service require no follow up, simply redirect them to
a Thank You page. If a customer was dissatisfied with
any aspect of customer service, send them to an apologetic
"I'm Sorry, you weren't completely satisfied" page
and follow up immediately, with a live operator
to salvage the customer relation.
How Does Free Form Generator
Work?
Basically, you design questions to
qualify the respondent into categories. The php
code automatically, sorts results into one of three
categories:
- Positive Response
- Negative Response
- Neutral Response
You assign separate locations
for posting results and redirects for each category.

Free PHP Form Generator Rules
There had to be a set of rules for the
code to sort results. We tried to keep the rules simple
but allow for maximum flexibility. The only limitation is your
imagination when designing and using Free Form
Generator.
There are only two types of
input: Text Box or Radio Buttons.
Text Box input is alpha and/or numeric as
determined by the respondent. These answers are always
a NEUTRAL value.
RADIO BUTTON answers allow the respondent
to pick only one best answer from a list.
Each answer choice can be assigned POSITIVE, NEGATIVE or
NEUTRAL values.
Each question may have any number of
NEGATIVE or NEUTRAL answers.
A question may have ONLY
ONE POSITIVE answer.
As you will learn, you may choose to have NO POSITIVE or NO
NEGATIVE answers for some questions.
Form results are sorted as follows:
A NEGATIVE submission
= ONE or more NEGATIVE
response selected.
A POSITIVE submission =
ALL POSITIVE responses
selected.
A NEUTRAL submission = all neutral or
combination of positive and neutral.
By default, the only required fields are
name and email.
If a form is submitted with only the
default required fields the results are sorted to the NEUTRAL
category.
Here are a few pointers to help you:
Lead generation

A negative
response should be an absolute "deal killer". If only one of
these are selected they post to the negative category. This
will save you time by filtering the unlikely leads.
Additionally, you may sell these leads to some other industry.
Example... I buy pre foreclosure homes. I ask "what is your
objective?", if they reply, "Save My Home and continue to live
in it".... That's a "deal killer" for me but a possible
prospect for someone who does mortgage
modifications.
Positive values
should be kept minimal, remember, a form must have ALL positive
values selected to be sorted as a positive submission.
Expanding on the fore-mentioned example,
"What is your objective?"
"Save my home and cont. to live in it"..... NEG "deal
killer".... sell lead "Not sure. Exploring all my options".....
NEU..follow up with auto responders
"Sell my house".....Send to PATLIVE, they call the lead
immediately and schedule an appointment for me to meet this HOT
prospect.
